Document Services

Document requests are considered on a case-by-case basis and completed only when clinically appropriate. Turnaround time: 7 days. Please note the minimum completed-session requirements before requesting documents.

Service Fees

Disability Forms Completion - $60

Any disability forms that have to be completed for the employer or insurance companies. The client must have at least 5 sessions completed before requesting. All forms will have a 7-day turnaround time to be completed.

Letters of Treatment Verification - $40

Any letters of treatment Verification. The client must have a minimum of 5 sessions completed before requesting. All forms will have a 7-day turnaround time to be completed.

Emotional Support Animal Letter - $50

Any Emotional Support Animal Letter. The client must have a minimum of 6 therapeutic sessions completed before requesting. Please provide the request with the animal’s name, breed, color, etc. All forms will have a 7-day turnaround time to be completed.

Reasonable Accommodation Letter - $35

Any reasonable accommodation letter or form for the employer. The client must have a minimum of 6 sessions completed before requesting. All forms will have a 7-day turnaround time to be completed.
